Our office strives to see each patient on schedule. At your initial appointment, please arrive 30 minutes before your scheduled appointment. Your initial appointment takes more time than a follow-up appointment. Our automated appointment reminder system will confirm your actual appointment time, however, your arrival or check in time is 30 minutes before your actual appointment time. You will be asked to reschedule if you are late.
We respect your time and patience. If you have been waiting greater than 30 minutes, kindly notify one of our practice administrative specialists for an update to your provider.
Your appointment time with your provider is dedicated to you. In consideration to all of our patients and for the safety of your child and staff, we ask that appropriate childcare arrangements be made for any child over the age of 6 months or you will be asked to reschedule. We appreciate your understanding.
We ask that you do not bring food and drink inside the office. Please consume them outside before you come in. There is a water fountain available in our reception area.
Visitors are limited to one adult per patient in the exam room. All others will be asked to wait in the reception area.
Please silence your phone at check-in and upon entering our clinical area as to not disturb any other patients or providers.
Payment is expect at the time of service unless prior arrangements are made. To find out if we are covered by your insurance or any fees associated with your appointment, please contact your insurance company prior to your visit. If you are uninsured and unable to make your full payment, please ask to see or speak with the Financial Counselor prior to your visit.
There is a charge of $25 for completion of FMLA, disability or leave of absence forms.
Patients are given a courtesy copy of their records at no-charge. There will be a $25 charge for any additional request.
If you need to cancel/reschedule your appointment for any reason, please provide at least a 24-hour notice in advance of your appointment date. Appointments cancelled or reschedule after that time will be charged a fee. Times are allocated based on the nature of your visit and the non-notification fees are as follows:
Patients with 3 no-show appointments will be dismissed from the practice.
At Northside Gwinnett OB/GYN, we are committed to keeping our patients, visitors and staff safe. In order to do this, we have a zero tolerance for verbal or abuse, or violent behavior. We reserve the right to contact the police and to remove anyone from the premises who engages in any of the following behaviors:
